To obtain a Pharmacist Registration Certificate (Green Card / Smart Card) from the Uttar Pradesh Pharmacy Council, applicants must satisfy the following statutory conditions under Section 32(2) of the Pharmacy Act, 1948:

  • Academic Qualification: Candidate must have passed Diploma in Pharmacy (D.Pharm), Bachelor of Pharmacy (B.Pharm), or Pharm.D from an institution approved by the Pharmacy Council of India (PCI) under Section 12 of the Pharmacy Act, 1948 for the passing year.
  • Mandatory Practical Training (500 Hours): For D.Pharm candidates, successful completion of 500 hours (not less than 3 months) of practical hospital/dispensary training under a Registered Pharmacist in a Govt / District Hospital or PCI-approved institution is strictly required. Training certificate must be in Form-III (Appendix-E).
  • Age Limit: Applicant must have attained the age of 18 years on or before the date of application.
  • Domicile / Residence: Candidate must be a resident of Uttar Pradesh (possessing a valid UP Domicile Certificate issued within the last 6 months) OR must have completed their pharmacy education from an approved institution located within Uttar Pradesh.

Step 1: Account Registration on Official UPPC Portal
Visit apps.uppc.in/online/Account/Registration. Enter your active mobile number, valid email ID, and create a secure password. Verify via mobile OTP to generate your login credentials.
Step 2: Fill Personal, Educational & College Details
Log in to the candidate dashboard and select “Request for Pharmacist Registration”. Enter High School (10th), Intermediate (12th), and D.Pharm/B.Pharm marks, passing year, roll number, and PCI college approval code.
Step 3: Upload Mandatory Scanned Documents
Upload clear, high-resolution scanned PDFs (under 1 MB each) of all original marksheets, degrees, 500-hour training certificate, domicile, photo, signature, and notarized affidavit.
Step 4: Online Fee Payment
Pay the prescribed registration fee of ₹1,829 through the payment gateway. Download and print the fee receipt and the submitted application form with barcode.
Step 5: Schedule Document Verification Appointment
Once fee is confirmed, the system enables the verification slot booking calendar. Select an available date and time slot for visiting the UPPC office in Lucknow.
Step 6: In-Person Physical Verification at Lucknow
